Shipper’s Declaration (IATA Section 8)

A description of the Shipper's Declaration that shippers must use when shipping dangerous goods under the IATA Dangerous Goods Regulations.

UPS Shipper's Declaration

When opting to comply with regulations promulgated by IATA, a Shipper’s Declaration must accompany each dangerous goods Parcel l or shipment unless shipping a multi-piece shipment to one consignee, in which case the shipper may either place a shipper’s declaration on every Parcel in the consignment, or provide shipment-level information on a Shipper’s Declaration on the lead Parcelof the shipment. Each DG Parcelmust have DG information assigned in WorldShip or the UPS-compatible shipping software used by the shipper.

UPS requires three original Shipper's Declarations on each hazardous materials Parcelor if a multi-piece shipment as described above, three original Shipper’s Declaration on the lead Pacel.
If more than one hazardous material is in a Parcel, one shipper's declaration must be used listing all materials. UPS allows a maximum of three compatible hazardous materials in one Parcel.

The following information must appear as indicated (references to IATA):

Shipper: Enter the shipper's full name and address – 8.1.6.1.
Consignee: Enter the full name and address of the consignee – 8.1.6.2.
Air Waybill Number: For air cargo shipments, the air waybill number is entered in this field. For small ,Parcelshipments, the tracking number is entered – 8.1.6.3.
Page X of X Pages: Enter the page number of the shipper’s declaration. If there is only one page for the shipment, note that the shipper’s declaration is ‘Page 1 of 1 Pages’ – 8.1.6.4.
Aircraft Limitation: Delete the type of aircraft that does not apply – 8.1.6.5.
Departure Airport: Leave blank.
Destination Airport: Leave blank.
Shipment Type (Radioactive/non-Radioactive): Delete the shipment type that does not apply – 8.1.6.8.
Nature and Quantity of Dangerous Goods – 8.1.6.9.
- UN or ID Number: Enter the UN or ID number for the material.
- Proper Shipping Name: Enter the proper shipping name. (with Technical Name when required)
- Class or Division: Enter the hazard class or division for the material.
- Subsidiary Risk: Enter the subsidiary risk, if applicable. This subsidiary risk must be enclosed in brackets.
- Packing Group: Enter the Packing Group assigned, if applicable.
- Quantity and Type of Packing: A description of the Parceland an indication of the quantity of material being shipped (use net quantity, unless the regulations require gross mass; metric measure is mandatory)
- Packing Instructions: The IATA Packing Instruction number must be noted in this column.
- Authorisation: Location for any Special Provision numbers, DOT special permits, LAA approval numbers, EX numbers or other pertinent authorisation information.
Additional Handling Information – 8.1.6.11: The shipper uses this box to indicate the appropriate 24-hour emergency response telephone number (in conjunction with the ER Registrant information) or any special handling information relevant to the shipment.
Certification – 8.1.6.12: By completing this section, the shipper certifies that the Parcelis in compliance with all rules and regulations.

The shipper is responsible for ensuring that all copies of the shipper's declaration are clear and legible.
