Commercial Invoice
Invoice Form If your shipment contains commodities to be purchased or sold or other non-document shipments, prepare three copies of a Commercial Invoice - or another document, such as a pro forma invoice, if an invoice is not applicable. Business or personal stationery may be used if it contains all the information on the invoice.

Required items include:

 Name and address of shipper (seller) as shown on the address label. Include contact person and telephone number.
 Name and address of consignee (ship to), including destination country and postal code, as shown on the address label. To help ensure prompt delivery, contact person and telephone number must be included.
 Invoice date.
 Purchase order or invoice number, if applicable.
 Name and address of purchaser (importer) if different from the consignee. Include contact name and telephone number.
 Complete description of each item being shipped. What is the item?
 What is it made of? What is it used for? (Include Customs Harmonized Codes, if known.)
 Country of origin (where manufactured).
 Number of units, unit value, and total value of each item. For samples or articles of no commercial value, a nominal or fair market value must be stated for customs purposes.
 Transportation charges and insurance.
 Total value of the shipment, including currency of settlement.
 Reason for export. For example: sale, repair, inter-company.
 Terms of sale (Incoterm) that define the charges included in the total value on the invoice. For example, C.I.F. is Cost, Insurance, and Freight.
 Number of packages and total weight.
 Shipper's signature and date.

Note: The Destination Control Statement is needed on the invoice for commodities that require a license or license exception. (See U.S. Shipper's Export Declaration for details). We recommend you record the statement on your invoice for all shipments.
