Note: Orders for preprinted forms cannot be combined with orders for other UPS supplies.

How To Order Preprinted Forms
|
- Once you are logged in to My UPS, select Shipping in the area at the top.
- Select Get UPS Labels, Paks and More in the area at the left.
- Select the Preprinted Forms image. (If you haven't ordered preprinted forms within the last 90 days, or if your My UPS profile does not contain an account number, the system will ask you to enter your Account Number and Postal Code first and then select Continue. If the Ship To Information is correct, select Continue, or edit the information, then select Continue.)
- Browse for the form you wish to customize and select Customize Form.
- Enter an order quantity in the quantity field.
- Fill out any fields you wish to customize, and select any corresponding options.
- Select Continue.
- Select Submit.

How To View Your Previous Orders

- Once you are logged in to My UPS, select Shipping in the area at the top.
- Select Get UPS Labels, Paks and More in the area at the left.
- Select the Preprinted Forms image.
- Select Details adjacent to Previous Order.

How To Reorder Previous Items
|
- Once you are logged in to My UPS, select Shipping in the area at the top.
- Select Get UPS Labels, Paks, and More.
- Select the Preprinted Forms image.
- Select Details adjacent to Previous Order.
- Select the item you wish to reorder.
- Select Reorder Previous Items.
- To review or edit the customized information before submitting your order, select More under Current Order Form Details.
- Edit the information as necessary and select Continue.
- Select Submit.

How To Switch to an Alternate Account Number
|
- Once you are logged in to My UPS, select Shipping in the area at the top.
- Select Get UPS Labels, Paks and More in the area at the left.
- Select the Preprinted Forms image.
- Select Edit adjacent to Ship To Information.
- Select Use Alternate Account Number.
- Select Yes.
- Enter the alternate UPS Account Number and Postal Code.
- Select Continue.
- If you'd like to keep the same Ship To Information, select Continue. If you'd like to change the information, edit the appropriate fields then select Continue.
