During the enrollment process you will be required to provide the following:
  • First name
  • Last name
  • Home address
  • Date of Birth

After successfully answering the security questions, your identity is authenticated and you'll receive your UPS My Choice® enrollment confirmation.

Note: Date of Birth information will not be saved or stored.
