Preprinted Forms
Refer to the helpful instructions below to order UPS preprinted forms.

Note: Orders for preprinted forms cannot be combined with orders for other UPS supplies.


How To Order Preprinted Forms
  1. Log in to My UPS.
  2. Select Shipping in the area at the top.
  3. Select Order Supplies in the area at the left.
  4. Select an authorized shipper account number from the drop-down menu.
  5. Select the Preprinted Forms category.
  6. Find the form you wish to customize and select Customize This Preprinted Form. You must customize at least one preprinted field to continue.
  7. Select an order quantity in the drop-down menu.
  8. Fill out any fields you wish to customize, and select any corresponding options.
  9. Select Order.

How To View Your Previous Preprinted Form Orders
  1. Log in to My UPS.
  2. Select Shipping in the area at the top.
  3. Select Order Supplies in the area at the left.
  4. Select Order History in the area to the left to view previous orders.

How To Reorder Preprinted Forms From Your Quick List
  1. Log in to My UPS.
  2. Select Shipping in the area at the top.
  3. Select Order Supplies.
  4. Select the Preprinted Forms category.
  5. Review your Quick List, and select the item you wish to reorder.
  6. Choose the desired quantity from the drop-down menu.
  7. Select the Update Your Supplies Order check box next to desired items.
  8. Select Add Selected Item to Your Order.

How To Switch to an Alternate Account Number For Your Preprinted Form Order
  1. Log in to My UPS.
  2. Select Shipping in the area at the top.
  3. Select Order Supplies in the area at the left.
  4. Select the Preprinted Forms category.
  5. If you would like to change the information, select Edit Ship-To Address.
  6. If you would prefer to keep the same Ship-To Address, select Continue.
  7. If you would like to change the account, select Use Alternate Account Number.
  8. Select your UPS Account Number from the drop-down menu.
  9. Edit the appropriate fields, then select Continue.
