Note: Orders for preprinted forms cannot be combined with orders for other UPS supplies.

How To Order Preprinted Forms
|
- Log in to My UPS.
- Select Shipping in the area at the top.
- Select Order Supplies in the area at the left.
- Select an authorized shipper account number from the drop-down menu.
- Select the Preprinted Forms category.
- Find the form you wish to customize and select Customize This Preprinted Form. You must customize at least one preprinted field to continue.
- Select an order quantity in the drop-down menu.
- Fill out any fields you wish to customize, and select any corresponding options.
- Select Order.

How To Reorder Preprinted Forms From Your Quick List

- Log in to My UPS.
- Select Shipping in the area at the top.
- Select Order Supplies.
- Select the Preprinted Forms category.
- Review your Quick List, and select the item you wish to reorder.
- Choose the desired quantity from the drop-down menu.
- Select the Update Your Supplies Order check box next to desired items.
- Select Add Selected Item to Your Order.

How To Switch to an Alternate Account Number For Your Preprinted Form Order

- Log in to My UPS.
- Select Shipping in the area at the top.
- Select Order Supplies in the area at the left.
- Select the Preprinted Forms category.
- If you would like to change the information, select Edit Ship-To Address.
- If you would prefer to keep the same Ship-To Address, select Continue.
- If you would like to change the account, select Use Alternate Account Number.
- Select your UPS Account Number from the drop-down menu.
- Edit the appropriate fields, then select Continue.
